I agree to permit Marc Chilianis (Sewing Machine Technician/Owner) to transport, service, and repair my sewing machine. I release Marc Chilianis and staff from all liabilities.

 

I understand that Marc Chilianis will notify me when my machine is ready for pick up and I will pick up my machine within 7 days unless other written agreement is made.

I understand that I will be invoiced when the Services are complete and are due upon receipt by cash or check: payable to MARC CHILIANIS. 

 

I understand if I leave my machines for more than 30 days it will be considered abandoned. If I fail to redeem my machine as agreed, I authorize Marc Chilianis to sell my machine to cover service expenses.

Cancellation: I understand and agree to give 24 hour notice to cancel or reschedule my appointment, so other clients have the opportunity to schedule in opening time period.

The first time a client misses an appointment, Marc Chilianis will call to reschedule client's appointment. If attempts to communicate with client are unsuccessful, all future appointments will incur a $30 non-refundable deposit.

Limited Warranty: For repairs, parts and service provided by us, we warrant to you that the repairs, parts and service we provide to you will be free from defects in material and workmanship. The duration of this warranty is thirty (30) days from as applicable the date of your receipt of the part, or the date of repair or service. If during this thirty-day period you discover a defect in the repairs, parts or service you must promptly notify us in writing. Marc Chilianis' obligation for defective parts and/or workmanship, and Customer’s exclusive remedy, shall be limited, at Marc Chilianis' option, to the replacement of any defective parts or workmanship or the refund of amounts paid by Customer for said the service and parts. All warranty claims are subject to inspection and approval by Marc Chilianis.
